Q3. How do I apply or renew my health (OHIP) card, and where do I go?

In order to apply for a Health Card you must attend in person and have the following documentation available:
Documentation providing proof of Ontario residency e.g.) valid Ontario Driver's License or temporary license, a utility bill or a mortgage/lease agreement, income tax assessment, property tax bill, CPP statement of contributions
Personal information with your name and signature e.g.) SIN card, student identification, library card, passport, driver's license, credit card, bank card, Ontario MNR Outdoors Card
Canadian citizenship or immigration status document e.g.) birth certificate, passport, certificate of baptism or equivalent if born in Quebec, immigrant visa or record of landing, certificate on Indian status
You and all members of your family, including your children, must each have a Health Card. Your Health Card should be carried with you at all times so that you can show it every time you may need health services.
Q4. How do I find out which doctors are accepting new patients in Haldimand-Norfolk-Brant?
Answer:
Contact the College of Physicians and Surgeons at: 1-800-268-7096. The college can mail you a list of area doctors, as well as their phone numbers, that are accepting new patients. It is then up to you to contact the doctors to set up an appointment.

Q10. What role does the provincial government and MPP Barrett play in local schools and school boards?
Answer:
The Government of Ontario is responsible for providing funding for education and for setting the curriculum and standards for student achievement. The school board is responsible for the administration of education, including busing.
Municipally-elected school board trustees are your advocate/representative when you have a concern or issue with the school board.

Q13. What is the Page Program at Queen's Park?
Answer:
Legislative Pages are Grade 7 and 8 students who work in the Legislative Chamber at Queen's Park in Toronto. The program is open to those in Grades 7 or 8 who are high achievers and are involved in community and school life. Pages receive a leave of absence from school during their Page term, which can last between three and six weeks.
During the term, Pages meet with politicians and officials, learn how laws are made and how Ontario is governed, participate in the legislative process and act as messengers in the Chamber.

Applications must be received no later than June 14 for the Fall session and December 13 for the Spring session. Students in Grade 8 must submit applications no later than December 13 of the year they enter Grade 8.
